The Cancer Patient Experience Survey is for patients who have received an invitation to take part. If you have received a survey in the mail, we would like to hear about your experiences of cancer care and treatment. If you would prefer to complete the survey online, you can do so by clicking on the ‘Take the survey online’ button below or scanning your personal QR code on the front of the questionnaire. You can enter your access code as it appears on your letter.
